At first I just had the messages Spam Assassin detected as spam flagged, and went through and deleted them manually. After 4-5 months I was pretty sure the system didn't delete any non-spam, so I made it delete them automatically, but keep logs of what is deleted (with subject and sender). I go through the logs now and then just to be safe, and so far I've only seen spammy subjects and unknown senders. It works really well.
