When player is dying and everything is fading, image overlays also have their opacity changed to disappear together with the rest of the room.
Unfortunately after the death animation finishes or is interrupted by undoing the changed opacity remains creating visual issues.
This can be seen in the hold I attached to the other report I made:

If you die to the roach you can see the image fade out (in 5.1). Interrupt the fade by undo and you'll notice the explosion is still partially transparent.
Let the death fade play till the end and the game will automatically undo, leaving the image completely invisible.
