I experienced this bug while playing a Stalwart room in Suwako Prizes in 5.0.2.1005. Here's how to recreate it:

-Place a stalwart and some roaches, or another monster that kills just like it does.
-The monsters will gang up on the stalwart but won't actually be able to kill it.

Adders and sworded monsters among others seem to still kill monsters, which narrows down the cause to monsters that kill by stepping onto a tile.

Stalwarts now back to dying like chumps to monsters that body kill. Confirmed fixed for Build 5.1.0.1006.
