An "example hold" is a small (usually one or two room) hold build in the editor that sets up a situation where the bug occurs. This makes it slightly easier to investigate, compared to having to look at certain rooms in real holds.
I have never worked in the editor, besides I am not sure if I'll be able to recreate this situation since I'm not sure when and why this is happening.
Does undo work in other situations? Your second screenshot shows the result before the undo.
And there's something wrong with the game over code, since you should have been automatically sent back to the last checkpoint after the death animation.
Also check your settings for "Auto Undo on Death", "Undo Level" and which key is bound set for undo.
No, second screenshot is after I've pressed the undo button. Look closely, there is a roach on top of the Beethro on the north. So when the death animation starts, I've usually push the undo button and it returns me to the previous move. It works in the majority of rooms, but sometimes (I've noticed that this is happening more often with the room of clones) it resets me to the state when I can't do anything (which makes these rooms really really irritating).
Also I've noticed that the windows sound notification happening during this issue, hovewer no error messages appearing.
The same issue is happening if I enable auto undo on death - the roach on top of the Beethro and I can't do anything.
