Nope, it's true for both, due to the structure to the hand, your fingers must go inside your hand, so therefore, the fingers on the smiley cannot be on the outside.
Trust me, I spent 15 minutes testing my theroy.

Scientifically, I'm not sure 15 minutes is enough to test a theory. In any case, I just replicated this guy :thumbsup in the mirror. It's an awkward hand position, but if you start with your hands open, palms facing away from you and thumbs toward the sky, then curl your fingers into a fist, you get what you see here. It looks weird because I think nobody ever does it like that; it's much easier to do the palms-facing-yourself thumbs-up.

the palms-outward version is more natural if your arms are extended from your body. the arms are elided because ... well, because it's just a smiley.
