I want to create a custom accessory using Roachie graphics, so I made a character based on Roachie and use Equipment Generate Accessory to place it in a room. Both on the floor and in the UI, it appears as a treasure chest, presumably because it's looking for non-directional graphics and Roachie doesn't have any.

I suggest that in this case, the South-facing graphics should be used.

I don't think this change would be desirable.

Every character, including equipment, has to be based off of a default entity type, like Roach or Wubba etc. Equipment shows the treasure chest when it has no custom graphics. The alternative would be to show the graphics of the base type, which would mean they would all show as roaches or wubbas etc. without custom graphics.

Given Roachie is probably the only use case people really have for this, it seems a lot easier to just have people provide their own Roachie sprite. I think the treasure chest is the much better default scenario than the graphics of the underlying type.
