I think I know why this is happening. Some of the monster behaviors were still buggy in the last version of DROD (e.g. serpents don't keep going forward when you're behind them, and brain guidance was slightly flawed -- no, I didn't make it harder, just fixed a buggy situation). Naturally, this will break some of the parts shown in the end sequence. I think I've handled those situations, however, and the ending should play fine in build 21.

Incidentally, this is also the reason why some people have been noticing that certain checkpoint saves seem messed up in their restore screen. This isn't a bug -- just a symptom that monster behavior has changed and the checkpoint saved game for this room is no longer valid, so they had to start at the beginning of the room here. This is necessary to release this version without a lot more delay, but should be fixed for later versions if existing monster behavior is tweaked any further.
