All those - Nintendo posted on april 1st a few years ago now that the Triforce was a hidden item that went in the centre of the six Medallions on the quest screen, and this was why the game let you continue on after you beat Ganon.
They posted a bunch of fake screenshots, and there were enough gullable people that didn't notice that the Triforce design wasn't actually a hole to perpetuate it around on the Internet, despite plenty of denials from both Nintendo and the game's scriptwriter (who, it's assumed, would have writen
something about finding the Triforce).
Never heard how Luigi in SM64 came about, but it's probably possible as Luigi was initally in the game, but was cut due to development issues.

×Reviving Aeris has become a running joke now amongst those who cheat, due to the sheer amount of people who believed this was possible (probably as a coping mechanism when the story knocks off Aeris without much warning).
I recall also that there were a lot of 'cheats' for finding secret pokémon in the first versions of the game. For the record, the Yoshi one actually worked, as I recall.
