Settings aren't tweaked (this is an in-game screenshot as I was playing), but I've cropped the HUD out.

Good guess! It's from Morrowind!

It's from the Deep Orc Passage in Arkngthand. Fans of the game will notice that's actually a cave - you get to the point on the screenshot by falling through the world (which it's possible to do there), swimming around and taking a screenshot.
