You can actually complete this room in two ways, with or without the roach.

×...you can work out the sword orientation you need by the position of the roach when you leave the confines of the serpent's body.

×More specifically, you should be facing east, since the roach will be east of you when you leave the serpent.
Also, you need to get to the roach as efficiently as possible.
Once in the central area, clear of the serpent, all you need to do is use the trapdoors to get the serpent to pass you without being able to get you (note the spawn counter carefully. If you do it right, it should be around 15-20.
