This is taking place in "Inner Chambers - 5N3E.

Yep, I killed him.. went north and finished the game. Later when I found the achievement, I restored the game to the room before, entered it, let him live and went east.

I've tried this 3 times, but I still get no achievement.

You still have to finish the game after not killing him. Did you do this?

If by finish you mean that I have to get to the credits; I'm not quite sure if I did that or if I just went up the stairs. I'll give it a try. Either way, it would be nice to edit the description of the achievement to avoid such misunderstandings in the future.
